How must treatment costs, living costs and insurance be demonstrated?

Living costs: means of subsistence are proven by a bank account balance statement in the applicant's name or a confirmation from a statutory body of a legal entity on financial and material means of subsistence, in the amount of the subsistence minimum for each month of residence (twelvefold if residence exceeds one year). Insurance: a document confirming health insurance must be submitted to the police department within 30 days of receiving the residence document. The official source does not state how treatment costs must be shown.
