Which family relationships qualify?

Close family members: spouse; same-sex (registered) partner; parent who is primary carer of an EU citizen under 21; the EU citizen's or spouse's descendants under 21; descendants or ascendants of the EU citizen or spouse who are (or were just before entry) dependent on their support or care. Distant family members: other relatives supported by or in the household of the EU citizen, or needing the EU citizen's personal care for serious medical reasons, or a partner in a proven long-term partnership. The same relations to a Czech citizen with permanent residence also qualify.

Who may sponsor or bring the family member, and what immigration/citizenship status must that person hold?

The sponsor is an EU citizen, or a citizen of Norway, Liechtenstein, Iceland, Switzerland or the Czech Republic; the page also says conditions apply to some United Kingdom citizens. The applicant must be that person’s family member under the route rules.

What is important for proving marriage, partnership, parentage, custody, dependency or another qualifying relationship?

The applicant submits the original of a document proving family membership of an EU citizen, e.g. a marriage certificate, birth certificate, custody decision or adoption decision. Distant family members must be supported by the EU citizen, be household members in the country of origin, or need the EU citizen's personal care for serious medical reasons; a partner must provide evidence of the long-term partnership. What important financial, accommodation, insurance or maintenance responsibilities apply?

Close family members must provide proof of accommodation and medical insurance; distant family members must provide proof of accommodation, medical insurance for the entire stay (otherwise the permit validity is shortened) and proof of funds.

How does the application work at a high level, what rights does the family member receive, how long does the status last, and how can it become independent or longer-term where officially stated?

For family members staying over 90 days, close family members of EU citizens (and of Czech citizens who exercised free movement) receive a residence card for the expected duration of the EU citizen's stay, up to 5 years; close family members of Icelandic, Liechtenstein, Norwegian and Swiss citizens, of Czech citizens who did not exercise free movement, and distant family members receive a residence permit for up to 3 years. Permanent residence can be sought after 5 years of continuous temporary residence, or after 2 years for close family members where for 1 of those years they were family of a Czech citizen with permanent residence or of an EU citizen holding permanent residence.
