Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

A visa-free stay is not intended, except in some cases, for applying for a long-term visa or residence permit; those must be applied for at a diplomatic mission in the country of the travel document, residence or citizenship. Exceptions: a visa-free stay can be used to apply for an EU citizen's family member temporary or permanent residence permit, and to collect an already granted long-term or permanent permit within the visa-free period (usually 90 days). The 90/180 rule does not apply where the stay continues on a long-term visa or residence permit, a bilateral agreement, or an exceptional Czech-only Schengen visa. The visa-free stay itself cannot be extended.
