What exact activity, role or purpose qualifies?

The qualifying activity is European voluntary service for applicants aged 18 to 30, or voluntary service whose purpose is to gain practical or professional experience; under the Foreigners Act this counts as 'studies' for a long-term visa or long-term residence permit.

What organisation, institution, religious body, charity, teacher or host must support the activity?

A host organisation in the Czech Republic is required. The service must also be organised or coordinated by a legal person accredited for that purpose by the Czech Ministry of Education.

What proves the placement, role, affiliation, experience or cultural/religious/voluntary activity?

A volunteer agreement with the host organisation is a necessary document. The official proof may be a confirmation of voluntary work for a particular organisation and must include the date until which the work will be carried out.

How long can they stay, can they change host/activity, can family accompany, and can the status be extended?

The visa is valid for at most 1 year; the host confirmation must state the date until which the voluntary work lasts; to stay longer the holder must extend the visa in time or apply for a long-term residence permit. The official source does not state whether the host can be changed or whether family can accompany.
