Which family relationships qualify?

For this quota-based permanent immigration route, the qualifying relationships are full brother or sister, grandparent or grandchild of a Ukrainian citizen (quota category 4 on the State Migration Service page, with equivalent wording in the Law of Ukraine On Immigration).

Who may sponsor or bring the family member, and what immigration/citizenship status must that person hold?

The sponsor or anchor relative must be a Ukrainian citizen. The quota category is expressly limited to relatives "of a Ukrainian citizen" (State Migration Service) and "of Ukrainian citizens" (Law on Immigration). The official pages do not state any additional immigration or citizenship status requirement for the Ukrainian citizen sponsor, such as residence in Ukraine or age.

The application is made at a Ukrainian diplomatic mission or consular post abroad (place of permanent residence) or, for persons legally staying in Ukraine, at the SMS territorial unit. The immigration permit is valid for one year. A person living abroad is issued a long-stay visa on request; after arrival the immigrant must apply for a permanent residence permit no later than 15 working days before the long-stay visa (or permitted stay) expires; the permanent residence permit is processed within 15 working days and issued for 10 years. The official source does not state the rights received.
