What other major conditions apply, how is the status obtained, and what renewal, document-validity, retention or loss rules matter?

The D visa is obtained from a Ukrainian diplomatic mission abroad (Embassy or Consulate), applying not earlier than 3 months before the planned entry; it is issued as a multiple-entry visa for 90 days unless legislation or treaties provide otherwise; supporting evidence is one of the listed documents, e.g. a certified copy of the work permit or a copy of the immigration permit. The official source does not state renewal, retention or loss rules for the residence permit obtained after entry.
