What exactly is the official special programme/status and why does it exist?

For section 8(1)(a)/(b) persons (except section 8(1)(a) persons whose family has given security), the Minister responsible for Health must have agreed to their treatment and care at a health institution in Trinidad and Tobago. The permit is in force for a specified period not exceeding twelve months and stays execution of any deportation order while in force. The Minister may attach terms and conditions to entry under section 10(3) and cancel the permit if they are breached, and may at any time in writing extend, vary or cancel it. On cancellation or expiry the Minister may order deportation, with no right of appeal. The Act does not state whether family members can be included.
