A completed and signed visa form with the passport photo page and yellow fever card attached; a passport valid for at least six months before expiry with an unused page for endorsement; one recent passport-size photo; a yellow fever vaccination certificate; an invitation letter from the firm or organisation in South Sudan explaining the purpose of travel; and the organisation's South Sudan registration certificate or a copy of the applicant's contract with a South Sudan-based organisation.
