What exact activity, role or purpose qualifies?

The activity is voluntary, unpaid participation in a European Voluntary Service programme (an EU-supported volunteer programme), working for an approved organisation such as a foundation or association, for a stay of more than 3 months.

What organisation, institution, religious body, charity, teacher or host must support the activity?

The volunteer service must be performed in an organisational unit approved by the minister competent for internal affairs under Article 157g(5) of the Foreigners Act; without approval (or if approval is refused, not extended or revoked) the permit cannot be granted. The agreement must be concluded with an organisational unit that has its registered office in Poland.

What proves the placement, role, affiliation, experience or cultural/religious/voluntary activity?

A written volunteer agreement with the organiser, containing: description of the service, its duration, conditions of completion and supervision, hours, funds covering living and accommodation costs and the minimum pocket money, and necessary training; plus Annex 3 to the application signed by the person authorised to represent the voluntary service organiser.

Can the person receive salary, pocket money, accommodation, meals or reimbursement? Can they undertake other work?

Volunteering is without remuneration, but the volunteer may receive a stipend or have expenses reimbursed; the agreement must cover funds for living and accommodation costs and minimum pocket money. For other work, as a rule a valid work permit or a statement on entrusting work to a foreigner is required.

How long can they stay, can they change host/activity, can family accompany, and can the status be extended?

The permit is granted for the period needed to complete the volunteer work under the agreement, from more than 3 months to 1 year. It is not renewed automatically; a new application must be made before expiry. The holder must notify the voivode within 15 working days of ceasing the volunteer service. The official source does not state whether family members may accompany or whether the host can be changed.
