Which family relationships qualify?

Partners who lead a family life (within the meaning of the European Convention on Human Rights) with a Polish citizen: an informal relationship, a civil partnership, or a marriage concluded under the law of the foreigner's country of origin or residence that is not recognised by Polish law. Close personal ties similar to marital ties, genuine and lasting, must be shown.

Who may sponsor or bring the family member, and what immigration/citizenship status must that person hold?

The sponsor is a citizen of the Republic of Poland residing in Poland. The applicant must be a third-country national or stateless person who does not belong to the category of foreigners enjoying EU freedom of movement as family members.

What is important for proving marriage, partnership, parentage, custody, dependency or another qualifying relationship?

Evidence includes a copy of the Polish citizen's identity card or passport, the partner's declaration of residing in Poland at the given address, and evidence of family life such as common residence before the application, maintained direct/telephone/email contact, witness statements, marriage or civil partnership registration, common offspring or any other useful documents. There is no closed list; the evidence must show actual, permanent and close personal ties corresponding to marital bonds.

What important financial, accommodation, insurance or maintenance responsibilities apply?

The applicant must have health insurance (public health insurance or insurer confirmation of treatment costs in Poland) and a stable and regular income sufficient for themselves and dependants: PLN 1,010 per month with no dependants, or PLN 823 per month for themselves and each dependent family member.

How does the application work at a high level, what rights does the family member receive, how long does the status last, and how can it become independent or longer-term where officially stated?

The application is filed electronically via MOS if the foreigner is in Poland, or on paper outside MOS if abroad, to the provincial governor competent for the foreigner's place of residence in Poland (or the Polish citizen's place of residence if the foreigner is outside Poland); the Head of the Office for Foreigners is the appellate authority. The permit is granted for the period needed, more than 3 months and up to 3 years; it is not renewed automatically and a new application must be made before expiry. As a rule, work requires a valid work permit (or, for citizens of Ukraine, Belarus, Armenia or Moldova, a statement on entrusting work). The official source does not state a path to independent or longer-term status.
