Which family relationships qualify?

A genuine, duly documented marriage with a Polish national recognised under Polish law: registered in accordance with the law, non-polygamous, heterosexual, not exclusively religious, between persons with legal capacity to contract it, and not fictitious (not contracted to circumvent immigration rules).

    In order for the permit to be granted, a foreigner must remain in in a genuine and duly tocumented marriage with a Polish national, and that relationship must be recognised under Polish law. It must therefore be a union registered in accordance with the law, non-polygamous, heterosexual, not exclusively religious in character, of persons having the legal capacity to contract it. The union cannot be fictitious – it cannot be contracted or exist for the purpose of circumventing the provisions defining the rules and conditions for the entry of foreigners to Poland, their transit through that territory, stay thereon and departure therefrom.

Who may sponsor or bring the family member, and what immigration/citizenship status must that person hold?

The sponsor is the Polish-citizen spouse: the marriage must have lasted at least 3 years before the application, and immediately before applying the foreigner must have resided continuously in Poland for at least 2 years on a temporary residence permit granted for this marriage or for refugee status, subsidiary protection or humanitarian stay.

What is important for proving marriage, partnership, parentage, custody, dependency or another qualifying relationship?

An up-to-date copy of the marriage certificate (the governor may require an Apostille or consular legalisation if its authenticity is in doubt), a copy of the spouse's identity card, and documents confirming uninterrupted stay or reasons for interruptions.

How does the application work at a high level, what rights does the family member receive, how long does the status last, and how can it become independent or longer-term where officially stated?

The application is submitted electronically through MOS while in Poland (applications from abroad are not permitted); the applicant then awaits a summons from the provincial governor to appear in person at the provincial office. It is decided within 6 months of a complete application. Continuity allows no single break longer than 6 months and breaks totalling no more than 10 months over the qualifying period (e.g. 2 years for a spouse). The permit is indefinite with a 10-year residence card; it allows work on the same terms as Polish citizens; it expires on obtaining EU long-term resident status or Polish citizenship, and leaving Poland for more than 6 years is a ground for revocation.
