Which family relationships qualify?

A child of a third-country national or stateless person holding a permanent residence permit or EU long-term resident permit in Poland, under that parent's parental authority, born after the parent was granted that permit, during the validity of the parent's temporary residence permit, or during the parent's stay on humanitarian or tolerated-stay consent or with refugee status/subsidiary protection.

How does the application work at a high level, what rights does the family member receive, how long does the status last, and how can it become independent or longer-term where officially stated?

The application is submitted electronically through MOS while the child is in Poland (applications from abroad are not permitted), and the applicant then appears at an appointment at the provincial office. The requirement to apply during a legal stay does not apply to a minor child born in Poland after the parent's permit was granted or during the parent's prior temporary residence permit, and illegal stay does not bar initiating proceedings for a minor child born in Poland to a foreigner granted a permanent residence or EU long-term resident permit. It is decided within 6 months. The permit is indefinite with a residence card valid 10 years; it allows work on the same terms as Polish citizens; it expires on obtaining EU long-term resident status or Polish citizenship, and leaving Poland for more than 6 years is a ground for revocation.
