Where can protection be requested, and is this a direct application, border/in-country request, referral or other process?

The application is sent electronically via MOS with the necessary annexes and stamp duty, followed by an appointment at the provincial office to present a valid travel document, give fingerprints and a signature specimen; it cannot be submitted from abroad and must be filed while in Poland.

What happens after the request: registration, family handling, major support and rights where officially stated?

The applicant presents a copy of the decision granting refugee status, subsidiary protection or the relevant consent and documents showing uninterrupted stay; for refugees and subsidiary-protection holders the whole period of the international-protection procedure counts, even time in a guarded or detention centre. The application is decided within 6 months; the permit gives a 10-year residence card, full labour-market access and the right to family reunification.
