For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

No advance visa is needed: the route concerns travellers staying visa-free, and the extension procedure is available only to citizens of Argentina, Brazil, Chile, Honduras, Costa Rica, Nicaragua and Uruguay, third countries with which Poland has bilateral agreements on visa-free travel.

What maximum stay, rolling-period rule, validity and entry rules apply?

Visa-free stay is limited to 90 days within any 180-day period; for the seven listed nationalities it may be extended by 90 days under the bilateral agreements, and only once.

The extension application requires a copy of a travel document valid at least 3 months after planned departure, with at least 2 blank pages and issued within the last 10 years (original shown), and documents confirming the necessity, purpose and conditions of the extension (invitations, bank statements, reservations, etc.).

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

Apply in person to the provincial governor (voivode) for the place of stay no later than the last day of visa-free stay; no fee; application in Polish with personal and passport data and a justification. The extension is granted by a stamp in the travel document; stay is legal while pending. A refusal can be appealed to the Head of the Office for Foreigners within 14 days.
