What organisation, institution, religious body, charity, teacher or host must support the activity?

For a long-term visa the NGO must be registered with the Government of Pakistan, and missions refer applications for prior Ministry of Interior clearance.

Embassies may grant a one-month short-term visa to expatriates working with NGOs in Pakistan. A long-term visa requires the NGO to be registered with the Government of Pakistan and prior Ministry of Interior clearance. For extension, registered NGOs apply to the Ministry of Interior, which authorises RPOs to extend. DGI&P lists the INGO Visa as 3 months. The Paris mission states NGO/INGO officials, staff and their family members are eligible to apply. The official source does not state whether the host NGO or activity can be changed.
