What exact activity, role or purpose qualifies?

Christian missionaries carrying out missionary work in Pakistan qualify; entry visas are not granted to missionaries seeking to open new missions or strengthen existing ones engaged in proselytising.

What organisation, institution, religious body, charity, teacher or host must support the activity?

An organisation in Pakistan must invite the missionary worker; the invitation must come from a registered missionary organisation.

What proves the placement, role, affiliation, experience or cultural/religious/voluntary activity?

Proof consists of a letter from a missionary organisation certifying bona fide membership, biodata for the last 10 years, and copies of educational degrees/diplomas/certificates (with English translation if required).

Can the person receive salary, pocket money, accommodation, meals or reimbursement? Can they undertake other work?

The missionary organisation takes full responsibility for maintenance, boarding/lodging and return journey if repatriation is required. The official source does not state whether salary or pocket money may be received or whether other work is allowed.

DGI&P's Ministry of Interior table lists the Missionary Visa as 1 year with two re-entries, extendable in Pakistan. The missionary section says the Ministry of Interior authorizes missions to issue an entry visa for one year with one re-entry; missionary workers apply for extension at the nearest Regional Passport Office one month before expiry, and the RPO grants two years with 2 re-entries per year (not valid for India).
