If you will pay these costs yourself, you must provide your bank statements for the previous 3 months and, if you are employed, 3 recent payslips.
If you will be staying in a hotel or rental accommodation, you will need a hotel reservation or rental agreement for the entire duration of your trip.
If you will be staying in a medical institution, this must be stated in the declaration from the medical institution.
The application is made according to the procedure for the applicant's country of residence, in most cases in person (including babies and children); applicants not resident in the country where they apply must show why they cannot apply in their country of residence; the visa can be applied for between 6 months and 15 days before travel.
