What exact activity, role or purpose qualifies?

Under a single permit (GVVA): work in dance, classical music, opera, musical, theatre or stage in an occupation in the top segment of the IND list, e.g. actor, choreographer, conductor, concert master, dancer or musical artist, director, soloist music or opera, singer or musical artist, designer (scenic design, costumes, light, sound, video).

What organisation, institution, religious body, charity, teacher or host must support the activity?

An employer whose company or organisation is registered in the Commercial Register of the Dutch Chamber of Commerce (where required under the Commercial Register Act 2007), which first looked for an employee in the Netherlands and EU/EEA by registering the vacancy with UWV, and which arranges housing.

    Your employer’s company or organisation is registered in the Commercial Register (in Dutch: Handelsregister) of the Dutch Chamber of Commerce (in Dutch: Kamer van Koophandel or KvK). This applies only if this entry is required under the Commercial Register Act 2007 (in Dutch: Handelsregisterwet 2007). Your employer has first looked for an employee in the Netherlands and the EU or EEA countries. For this purpose, your employer must have registered the vacancy at an employer service point of the Employee Insurance Agency (in Dutch: Uitvoeringsinstituut Werknemersverzekeringen or UWV).

What proves the placement, role, affiliation, experience or cultural/religious/voluntary activity?

How long can they stay, can they change host/activity, can family accompany, and can the status be extended?

The single permit is valid for a maximum of 3 years (maximum 5 years after 5 years with a valid residence permit for work). A new employer must apply for a new residence permit; the permit can be extended. The official source does not state whether family members may accompany.
