Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

Eligible individual eVISA applicants use official MYVISA from outside Malaysia before travel. The general MEV page says extension of each 30-day stay is not allowed. Receiving a new eVISA cancels an older active visa, while rejection of the new application leaves the older visa valid; neither is an automatic stay extension.
