What medical treatment/purpose does the route cover and which applicant role does it address: patient, donor or another medical purpose?

Covers foreign patients visiting Japan for medical purposes: any acts instructed by medical institutions in Japan, including full medical check-ups, medical examinations, dental care and recuperation (including hot-springs recuperation of 90 days or less).

What acceptance, appointment, referral, treatment plan or healthcare-provider documentation is important?

The patient arranges medical services through a registered guarantor (e.g. medical coordinator or travel agency), and presents a "Certificate from a medical institution about the planned medical services and references from a guarantor" obtained via the guarantor.

The lists are at the end of this document. Foreign patients should contact one of the guarantors, and request arrangements for receiving medical services.

How must treatment costs, living costs and insurance be demonstrated?

Documents showing the foreign patient's financial situation (e.g. a bank statement) must be presented. The official source does not state separately how treatment costs, living costs and insurance must be shown.

Can a carer/companion accompany the patient? Who qualifies and what separate/linked permission applies?

Yes. Not only family members but also non-family members may accompany the patient if necessary. Accompanying persons may be issued the same visa as the patient; they visit to take personal care of the patient and may not engage in income-generating business or paid activities.

How long can the patient stay, what happens if treatment continues longer, and what official extension or follow-up mechanism exists?

Period of stay is 90 days, 6 months or 1 year, determined by the patient's clinical condition and other factors. If the expected stay is longer than 90 days, the patient must stay in a Japanese medical institution and obtain a Certificate of Eligibility from the Immigration Bureau through a staff member of the hospital or a family member living in Japan. The official source does not describe an in-country extension mechanism.
