Where can protection be requested, and is this a direct application, border/in-country request, referral or other process?

This is a direct application: the application form is printed and filled in, then the applicant attends in person (personal attendance is mandatory; attorneys with power of attorney may be used where the person cannot move) at the nearest Population and Immigration Authority office or the nearest Israeli mission. Documents: foreign passport valid for half a year beyond the permit period, two colour passport photos 3.5 x 4.5 cm, and National Insurance Institute confirmation of victim status.
