What other major conditions apply, how is the status obtained, and what renewal, document-validity, retention or loss rules matter?

ITAS-to-ITAP conversion is decided by Ministerial Decision; granting, extension and cancellation are done by the Minister or designated immigration officer. ITAP is granted for 5 years and can be extended indefinitely unless cancelled; holders of an indefinite ITAP must report to the Immigration Office every 5 years, free of charge. Holders get a Re-entry Permit valid 2 years (not beyond the ITAP). ITAP ends if the holder leaves Indonesia for more than 1 year or does not intend to return, does not extend after 5 years, acquires Indonesian citizenship, has it cancelled, is deported or dies.
