What maximum stay, rolling-period rule, validity and entry rules apply?

The D12 allows multiple entries with a stay of up to 180 days per arrival, counted from the date of arrival. The stay can be extended once, up to a total stay of 12 months (1 year) or 2 years depending on the visa duration chosen. Visa validity runs from the date of issue; if the visa is not used within that period, a new visa is needed.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

The visa is applied for online at evisa.imigrasi.go.id and processed by the Directorate General of Immigration, with a processing time of five working days after the visa payment is received. The stay can be extended once for a further 180 days; the extension can be made online by the sponsor through a registered account at evisa.imigrasi.go.id. The stay permit from this visa cannot be converted into a limited stay permit (izin tinggal terbatas).
