What exactly is the official special programme/status and why does it exist?

It is an Indian visa category listed in MHA visa guidelines with sub-categories for the UN official or diplomat assigned to India or on a visit to India, and their dependants (the "X" sub-categories). The official source does not state why the category exists.

The current Gaborone consular table lists a UN non-diplomatic official assigned to India under Official Visa O-3. This corresponds by role and purpose to the legacy UN Official Visa label; the 2018 Annex code UO-1 is not the current code in this table.

What circumstance, event, relationship, nomination, invitation or status activates access?

Access is triggered by UN non-diplomatic official status combined with an assignment or visit: UO-1 for a UN non-diplomatic official assigned to India; UO-2 for a UN non-diplomatic official on a visit to India.

What sponsor, authority, nomination, certification, evidence or other step must occur before the status can be obtained?

For foreign diplomats or officials assigned in India and their dependants, visa applications are submitted to the CPV Division of the Ministry of External Affairs via the e-FRRO online portal (new issuance in India only per bilateral agreements or for newborns; also extension, conversion, transfer). Applications should be made at least 3 weeks before visa expiry.

Family can be included: dependants of a UN non-diplomatic official assigned to India, including ordinary passport holders (UO-1 X), or visiting India (UO-2 X) have their own sub-categories, and ordinary-passport-holding dependants of officials assigned to India (including in the UN) may be granted an X-5D visa for appropriate duration.
