What maximum stay, rolling-period rule, validity and entry rules apply?

The visitor visa is valid for up to five specified places and up to 6 months, with each continuous stay not exceeding three months; up to two years' multiple entry applies for seniors over 65, a Pakistani married to an Indian national and their children under 12.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

Applications are made online in the standard visa application form; visitor-visa holders (unless exempt from police reporting) must register with the FRRO/FRO within 24 hours of reaching the place of stay and report 24 hours before leaving it. The official source does not state extension rules.
