What medical treatment/purpose does the route cover and which applicant role does it address: patient, donor or another medical purpose?

The route is for a foreigner whose sole purpose is medical treatment in established/recognised/specialised hospitals or treatment centres in India, including treatment under Indian systems of medicine; the applicant is the patient.

See recorded sources

What acceptance, appointment, referral, treatment plan or healthcare-provider documentation is important?

Hospital authorities must share the documents they issue, e.g. a medical invitation letter and appointment letter, in the Appendix II format on the official e-mail ID of the Indian Mission/Post concerned. Medical visa extension by the FRRO/FRO requires a medical certificate from a Government/ICMR/NABH/MCI/CGHS-recognised hospital.

How must treatment costs, living costs and insurance be demonstrated?

The checked official sources did not provide enough route-specific evidence to confirm how treatment costs, living costs and insurance must be shown.

Can a carer/companion accompany the patient? Who qualifies and what separate/linked permission applies?

Yes: attendants/family members may get a MED X visa co-terminous with the patient's visa; minor children accompanying the parent-patient may get an X-Misc visa. Under e-Visa, only two e-Medical Attendant visas are granted per e-Medical visa.

How long can the patient stay, what happens if treatment continues longer, and what official extension or follow-up mechanism exists?

Missions may grant a 6-month triple-entry Medical Visa by default (other rules for some nationalities). If treatment continues, the FRRO/FRO may extend for up to one more year on a medical certificate from a Government/ICMR/NABH/MCI/CGHS-recognised hospital; any further extension only by MHA.
