For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

An advance Schengen (uniform) visa is required only for third-country nationals who must hold a visa to cross the external borders under the EU visa list; Greece publishes a country-by-country table of visa-required and visa-free nationalities.

What maximum stay, rolling-period rule, validity and entry rules apply?

Stays of no more than 90 days in any 180-day period; the visa may be for one, two or multiple entries with a validity not exceeding 5 years; a single-entry visa includes a 15-day period of grace.

What important funds, accommodation, invitation, insurance, onward-travel or passport evidence may be required?

Documents showing the purpose of the journey, accommodation (or means to cover it), sufficient means of subsistence for the stay and return, and information on intention to leave before the visa expires; plus travel medical insurance covering repatriation for medical reasons, urgent medical attention, emergency hospital treatment or death.

    When applying for a uniform visa, the applicant shall present: (a) documents indicating the purpose of the journey; (b) documents in relation to accommodation, or proof of sufficient means to cover his accommodation; (c) documents indicating that the applicant possesses sufficient means of subsistence both for the duration of the intended stay and for the return to his country of origin or residence, or for the transit to a third country into which he is certain to be admitted, or that he is in a position to acquire such means lawfully, in accordance with Article 5(1)(c) and (3) of the Schengen Borders Code; (d) information enabling an assessment of the applicant’s intention to leave the territory of the Member States before the expiry of the visa applied for.

    Applicants for a uniform visa for one or two entries shall prove that they are in possession of adequate and valid travel medical insurance to cover any expenses which might arise in connection with repatriation for medical reasons, urgent medical attention and/or emergency hospital treatment or death, during their stay(s) on the territory of the Member States.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

Applications are made to the consulate of the competent Member State (Greece when it is the sole destination) in the jurisdiction where the applicant legally resides, normally no more than 6 months and no later than 15 days before the trip. An issued visa is extended free of charge for force majeure or humanitarian reasons, and may be extended for serious personal reasons for a EUR 30 fee.
