For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

Visa-requiring third-country nationals (not UK citizens) must hold a Schengen short-stay visa and meet the other applicable entry conditions; Schengen short-stay visas issued by any Schengen State are valid for entry into Gibraltar. Over 60 passport jurisdictions are visa-exempt, including the USA, Japan, Mexico, Brazil, Canada, Australia, Israel and the United Kingdom. The notice does not apply to Gibraltar identity cardholders, Gibraltar civilian registration cardholders or other British citizens.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

A Schengen short-stay visa is applied for to the appropriate Schengen authorities: where Gibraltar is the main destination, Spain issues it; where the main destination is elsewhere in the Schengen Area, the competent Schengen State does. Applications are processed under the Schengen Visa Code by the competent consular authorities or their authorised visa centres. UK visas for Gibraltar can no longer be applied for. The official source does not state when a short stay can be extended or changed.
