What exactly is this: asylum, temporary protection, humanitarian admission, resettlement or another protection mechanism?

A humanitarian admission mechanism: the supreme authority of a federal state (Land) may order, for reasons of international law, humanitarian reasons or German political interests, that a residence permit be issued (§ 23(1) AufenthG). Not an asylum procedure.

Where can protection be requested, and is this a direct application, border/in-country request, referral or other process?

Admission follows a state order, not an individual visa application; the order may require a formal declaration of commitment (§ 68) by a sponsor.

Spouse/child reunification only for international-law, humanitarian or political reasons. Humanitarian permits are issued or extended for at most three years at a time.
