What investment option or options qualify?

The real-estate option requires purchasing a unit of an Approved Project under the Citizenship by Investment Programme.

What official minimum amount, tier or threshold applies to each relevant option?

The unit must be purchased from an Approved Project for at least US$200,000. Government fees are separate: US$75,000 for the main applicant; US$100,000 for the main applicant and up to three dependants; US$25,000 for each additional dependant under 18; US$40,000 for each additional qualified dependant aged 18 or older; plus a processing fee of US$1,000 per application and due-diligence fees of US$7,500 for the main applicant and US$4,000 for dependants aged 16 or above.

How long must the investment be maintained and what continuing ownership, investment or physical-presence conditions are important?

The approved real estate must be held for three years from the date citizenship is granted, or five years when the future purchaser is also a Citizenship by Investment applicant. The official source does not state physical-presence conditions.
