The sponsor must be at least 21 years old and be either a US citizen or national.     The permit reaches you through your sponsor: after the Non-US 30-Day Permit has been processed and paid, the sponsor will be responsible for sending a copy to the traveler, and the traveler must hand-carry and present that copy to an immigration officer upon arrival. Traveler documents are identification such as a valid passport or document of identity valid at least 6 months after departure, a round-trip ticket showing travel within 30 days, police clearance from country of origin, and for minors travelling without a parent, notarized parent or guardian consent to travel with the birth certificate included. Without the approved permit you may be turned away at any of the Ports of Entry in American Samoa and be required to return on the same vessel you embarked on.

    The sponsor applies in person at the Immigration Office, not online: applications must be submitted in person within 3-5 business days prior to travel, and any application submitted later may not be processed.
